Brookfield Care Home

Brookfield Care Home provides 24-hour residential care to a maximum of 32 residents in Lymm, Cheshire. We welcome people on both a permanent and respite basis, our dedicated care team will ensure that you are well looked after. Residents have their own bedroom with adjoining en-suite bathroom. We have one double room suitable for couples that wish to stay together.

Our residents are supported with 24-hour care from trained Care Assistants and regular visits from GPs and District Nurses. All of our residents have their own personalised care plan, which is developed with the support of relevant healthcare professionals, family members and of course, the resident themselves. Regardless of whether you are living at Brookfield on a permanent or respite basis, the same thoroughness will be put into the planning and delivery of your care.

We support people over the age of 55 with a range of health conditions. Residential care can include support with the following: washing, dressing, toileting, assistance with eating, support mobilising and medication prompts. There is no one-size ‘fits all’ approach to care, our Care Assistants will tailor their care to you as an individual.

Staying mentally engaged and physically active is a huge part of staying well. Our Activities Coordinator listens to the requests of residents and plans activities accordingly. For example, upon resident request ‘paper mornings’ have been established, this is where residents come together to discuss current affairs. Also upon resident request, regular cheese and wine evenings have been established! Family members are very welcome to join us for all activities but food-related ones definitely appear to be the most popular!

Breakfast, lunch and dinner can be enjoyed with fellow residents in the dining room of the home. Meals are better when taken with friends, we find that by making meals an occasion it tempts residents with smaller appetites to eat well. Our Head Chef is supported by Nutrica, who provide practical workshops, giving the Chefs necessary knowledge and skills to meet the dietary requirements of residents on special diets.
